To Use Before and After the Program Starts
1. Understanding Needs
- What daily tasks do I need help with?
- What are my main goals (e.g., staying at home, being social, improving mobility)?
- Do I need short-term help, ongoing support, or both?
2. Planning Care
- Have I had a care plan meeting?
- Is my care plan written down and easy to understand?
- Does it reflect my preferences and cultural needs?
3. Money Matters
- Have I seen a full list of all costs?
- Do I understand care management and admin fees?
- Have I compared prices from more than one provider?
4. Staying Independent
- Have I asked about physiotherapy, occupational therapy, or nutrition advice?
- Do I know what home modifications could help me?
5. Local Connections
- Do I know my local council, community centre, or seniors’ group?
- Are there local programs that combine social and health services?
6. Staying Informed
- Am I signed up for My Aged Care updates?
- Do I know who to contact if I have a problem?
- Have I saved numbers for COTA and OPAN?
